Information Update Form

The electronic Information Update Form must be submitted if you have had changes to your home mailing address or school choice since submitting your Scholarship Acceptance Form.

This form must be submitted by May 31, 2024 to be updated prior to checks being sent. If updates are needed after May 31, 2024, the Check Request Form will become available July 1, 2024.

Upon receipt of this form Scholarship America will process your request accordingly.

Eligibility Requirements: Students must be enrolled at an accredited school for the Fall 2024-Summer 2025 academic year.
